REMEDIAL LAB is committed to providing quality services to you and this policy outlines our ongoing obligations to you in respect of how we manage your Personal Information.

We have adopted the Australian Privacy Principles (APPs) contained in the Privacy Act 1988 (Cth) (the Privacy Act). The NPPs govern the way in which we collect, use, disclose, store, secure and dispose of your Personal Information.

What is Personal Information and why do we collect it?

Personal Information is information or an opinion that identifies an individual. Examples of Personal Information we collect include: names, addresses, email addresses, phone and facsimile numbers.

This Personal Information is obtained in many ways including our boooking system timely, via our website www.remediallab.com.au, via contact form and from third parties. We don’t guarantee website links or policy of authorised third parties.

We collect your Personal Information for the primary purpose of providing our services to you, providing information to our clients and marketing. We may also use your Personal Information for secondary purposes closely related to the primary purpose, in circumstances where you would reasonably expect such use or disclosure. You may unsubscribe from our mailing/marketing lists at any time by contacting us in writing.

When we collect Personal Information we will, where appropriate and where possible, explain to you why we are collecting the information and how we plan to use it.

What personal information do we collect and how do we collect it?

The type of personal information we collect may include contact information (eg name, email address, postal address, company information, and other profile information), login details, comments, preferences, feedback, device identifiers and user data (see Cookies, Analytics and Advertising). 
We collect personal information in a number of ways, including directly from you, and when you provide it to us or our agents, contractors or distributors. For example, we will collect your personal information if you:

  • create a timely booking account with us, 
  • submit an enquiry, 
  • respond to a promotion by submitting your details, 
  • apply for employment with us, or
  • use our Website

 Personal information may be collected by way of: 

  • name,
  • age,
  • Date of Birth,
  • emails, 
  • phone number, 
  • medical history
  • forms filled out by individuals (including via online forms), 
  • feedback, 
  • billing information,
  • enquiries, 
  • support requests, 
  • surveys, competitions, 
  • face-to-face meetings and
  • interviews. 

We may supplement the information we receive from you with information from third party sources such as our booking system and service providers. 

You may in some circumstances have the option of not identifying yourself or using a pseudonym when you deal with us. However, if you choose not to provide the information we need to fulfill your request for a specific service or to participate in standards development activities we may not be able to provide you with the requested service.

Sometimes our activities require us to collect sensitive information. For more details, see the section ‘Sensitive information is subject to greater restrictions’, below.

When purchasing a services from REMEDIAL LAB you will be issued invoice online, through our timely booking app or in our treatment room:

  1. Customers will be directed to a payment gateway provider. 
  2. Payment card details are submitted to the payment gateway provider
  3. REMEDIAL LAB will receive certain information about Customers from the payment gateway provider (including payment card details in a  masked format and the Customer’s name and IP address) to enable REMEDIAL LAB to provide services ordered by Customers.

         

Cookies, Analytics and Advertising

REMEDIAL LAB uses cookies and device identifiers to identify visitors to the Website and Store, to improve our services, products and applications, to gather statistical and demographic information, store user preferences, activity, and session information, to improve security, enhance website and application functionality and show Standards Australia’s ads across the internet and devices.

REMEDIAL LAB also uses web analytics and session recording technology provided by third party service providers such as Google Analytics. These services use cookies to assist us in understanding how visitors access and utilise our Website and applications. 

A ‘cookie’ is a small file which contains a piece of text identifying your browser to our site. 

You can opt-out of REMEDIAL LAB’s use of cookies in your browser at any time by setting your browser to refuse all or some browser cookies, or to alert you when cookies are being sent. If you disable or refuse cookies, please note that some parts of the Website may then be inaccessible or not function properly.

In addition, REMEDIAL LAB uses the Google AMP Client ID API to consolidate information on each unique visitor to their many interactions with our Website.  This consolidated information provides a more accurate picture of visitor journeys and use of our Website. The Google AMP Client ID API assigns a unique randomly generated identifier to each visitor and uses this identifier to link all activity associated with that particular visitor.

The unique device identifiers, web and mobile analytics data and cookies do not to collect or contain personally identifiable information such as your name, email address or phone number. However, we may collect information about the devices you use, including, hardware model, operating system, version and internet protocol (IP) address. In some circumstances an IP address could be linked to an individual. Your IP addresses, device identifiers and cookies may be collected or accessed by our third-party service providers. 

You can manage how your browser and mobile device use data by adjusting your privacy and security settings in your browser and mobile device. 

Sensitive Information

Sensitive information is defined in the Privacy Act to include information or opinion about such things as an individual’s racial or ethnic origin, political opinions, membership of a political association, religious or philosophical beliefs, membership of a trade union or other professional body, criminal record or health information.

Sensitive information will be used by us only:

  • For the primary purpose for which it was obtained
  • For a secondary purpose that is directly related to the primary purpose
  • With your consent; or where required or authorised by law.

Third Parties

Where reasonable and practicable to do so, we will collect your Personal Information only from you. However, in some circumstances we may be provided with information by third parties. In such a case we will take reasonable steps to ensure that you are made aware of the information provided to us by the third party.

Disclosure of Personal Information

Your Personal Information may be disclosed in a number of circumstances including the following:

  • Third parties where you consent to the use or disclosure; and
  • Where required or authorised by law.

Security of Personal Information

Your Personal Information is stored in a manner that reasonably protects it from misuse and loss and from unauthorized access, modification or disclosure.

When your Personal Information is no longer needed for the purpose for which it was obtained, we will take reasonable steps to destroy or permanently de-identify your Personal Information. However, most of the Personal Information is or will be stored in client files which will be kept by us for a minimum of 7 years.

Access to your Personal Information

You may access the Personal Information we hold about you and to update and/or correct it, subject to certain exceptions. If you wish to access your Personal Information, please contact us in writing.

REMEDIAL LAB will not charge any fee for your access request, but may charge an administrative fee for providing a copy of your Personal Information.

In order to protect your Personal Information we may require identification from you before releasing the requested information.

Maintaining the Quality of your Personal Information

It is an important to us that your Personal Information is up to date. We  will  take reasonable steps to make sure that your Personal Information is accurate, complete and up-to-date. If you find that the information we have is not up to date or is inaccurate, please advise us as soon as practicable so we can update our records and ensure we can continue to provide quality services to you.
